Apollo 15 insurance cover, bottom cover
Because of new information from Apollo 15 astronaut Al Worden, it is now accepted by most that only the bottom type cover is an actual insurance cover and the only type of insurance cover used for Apollo 15. The top cover whas been demoted to just crew signed cover status.
